W-9

Form W-9 is used by Payees to file an information return with the IRS on reportable payments made to others. This form is never actually sent to the IRS. The company that files the information return keeps the form on file for verification purposes. W-9 forms also help the payee avoid backup withholding, depending on the exemptions available to the payee.

W-8

Form W-8 (or W-8BEN) is for foreign investors and grants an exemption from certain U.S. information return reporting and backup withholding regulations. It shows that the foreign investor is not subject to full taxation on investment income, but qualifies instead for a lower rate in accordance with tax treaties between the United States and his or her home country.

DRIP Enrollment Form

The DRIP Enrollment Form is used by Temper of the Times Investor Services to process orders for investors who wish to enroll in Direct Investment Plans. Each form has space for up to 18 different companies. Temper processes the order by buying the number of shares required for the customer to qualify as a shareholder for the purpose of opening a DRIP account. For most companies the number of shares required is one (but Temper will accept orders for up to 100 shares). The shares will be purchased in the customer’s name (not street name) and Temper will deal with the transfer agent on behalf of the customer until the account is open.

DRIP Enrollment Form (Gift)

The DRIP Enrollment Gift Form is used by Temper of the Times Investor Services to process orders for investors who wish to enroll a third party in Direct Investment Plans. Each form has space for up to 18 distinct companies.
